Nooksack Tribal Members
This form is required for COVID-19 testing at the Nooksack Health Clinic. Testing is available M-F from 9 am – 4 pm, with a lunch closure from 12-1 p.m. For questions or to schedule an appointment, please call the Health Clinic at 360-966-2106. This form is confidential and will connect you directly with a Nooksack Health Clinic nurse.
If you would like to obtain a copy of your test results, please complete and bring a copy of the Authorization for Release of Medical Records.
Important Update
The Nooksack Health Clinic has updated its policies on COVID-19 testing. If you would like a COVID test from the Health Clinic, testing must include the following and meet the approved testing criteria. Please Note: Insurance will no longer pay for asymptomatic testing. This includes Cigna, DSHS, Medicare, and HMA. If a COVID test does not fit the testing criteria as outlined and is NOT requested by medical provider or Nooksack Tribe entity, the patient will be responsible for payment.
